Please do not schedule this appointment if you have not previously completed an Initial Medication Reconciliation with us. This service is for amending the initial medication list after a hospital visit, urgent care visit, doctor visit etc. We will discuss medication changes, collaborate with your doctor for any needed clarification and email you an updated Medication list to share with family, caregivers, EMS and your doctors.
Cancellation Policy
One free consultation per person annually. Please cancel or reschedule at least 24 hours before the set appointment time if needed.
