Accurately identify current medications and comparing them to new orders after a doctor's visit or transition of care (e.g., admission, transfer, discharge) to avoid errors such as omissions, duplications, dosing mistakes. It involves reviewing all prescribed, over-the-counter, and supplement medications with the patient or caregiver and assistance resolving discrepancies with your doctor. A finalized copy will be emailed to you to keep handy for your family/ caregivers, EMS and medical providers. To make medication management safer and easier, talk to your pharmacy about having them provide your medications in blister packs.
